What to Bring
If you are coming in for a work injury or car accident, please bring in your WCB (Workers Compensation Board) or MPI (Manitoba Public Insurance)injury identification number. If you have it, the name of your adjudicator and their phone number are helpful, but not required.
If you have Blue Cross extended health coverage and would like us to bill them directly for your physiotherapy treatments, please bring in your group and contract numbers located on your card. You may have already provided this to the clinic when booking your assessment in which case it is not necessary to bring your card with you.
If your problem involves the feet, knees, or hips, it is helpful to bring in a pair of shorts. If the problem is in the upper extremity, it is helpful to wear a tank top. The clinic also carries spare shorts and gowns for those who need them. If the problem is in the back or pelvis, it is helpful to wear pants with an elastic waist.
If coming in for a vertigo assessment, please bring in a list of medications you are taking and arrange for someone to drive you home afterwards (the appointment takes approximately an hour).
